Electronic Visit Verification (EVV) Resources Page
 – Choice Home Care is rolling out Choice EVV, a secure mobile application for our employees to clock in and out of work in real time, with their phones, replacing paper timesheets.

 

Caregiver Forms
  • EVV Time Adjustment Form (For Regional Center client use only)
    • When issues arise that prevent the Choice EVV Mobile App  from being used, an adjustment to the shift or a manual time entry will need to be made using the  EVV Time Adjustment Form. Employees will need to fill out the form, and a member from the administrative team will need to review and approve the form.
  • JFS Time Adjustment Form (For Senior Care client use only)
  • Caregiver Time Sheet (Paper)
    • Caregivers can download Paper Time sheets here. Caregivers who are using EVV cannot submit Paper Time sheets and must use Choice EVV to Clock In/Out.
